The iPhone camera mis-parses vCard 4.0 and can swap first and last names. Version 3.0 is read correctly by every current phone.
A full contact is a lot of data. Fill in only what people need, print it at 3 cm or larger, or switch to MeCard for a smaller code.
Not in the code itself; a photo would make it unscannable. Add your logo as an overlay on the code instead.
